The Sports Medicine Journal – radio interviews with Dr. Michael Wright

Once a month, Colter Nuanez of ESPN Missoula is joined on “Nuanez Now” by Dr. Michael Wright, an orthopedic surgeon at Missoula Bone & Joint to discuss a variety of sports injuries that afflict athletes. Dr. Wright is a fellowship-trained, board-certified sports medicine specialist with an interest and expertise in arthroscopic surgery of the shoulder, hip and knee. Dr. Wright specializes in ACL reconstruction, rotator cuff repair, meniscus knee surgery, shoulder instability, hip labrum surgery, patellar instability, articular cartilage procedures, and shoulder replacement surgery. Dr. Wright joined the Missoula Bone & Joint team in 2017, after completing his fellowship training in Sports Medicine at UCLA and working with the LA Lakers. The focus of his practice is joint preservation. Dr. Wright works with his patients to explore all treatment options including non-surgical solutions such as physical therapy, oral medications, home exercise programs and injections.

Hip labrum and shoulder labrum injuries

Dr. Wright explains how structurally they are similar as they are both made out of fibrocartilage material. However, functionally they are quite different. In the shoulder the labrum plays an integral part in stability; the shoulder joint is a ball sitting on a saucer so it relies on the labrum to hold it together. Whereas the hip is a ball and socket joint so the labrum functions as a suction and seal to keep the joint lubricated. Injuries to the labrum in either joint can cause pain with activity. Labral tear injury to the shoulder is much more common. You will see injuries to the hip labrum with activities that require repetitive hip flexion: sprinters, jumping, soccer and hockey players. Hip labrum injuries present with deep pain in the groin, and catching and locking in the hip.
Dr. Wright discusses when to treat surgically, time frame for return to play and rehabilitation, spectrum of injury, the range of motion and strengthening phase.

Hamstring injuries

Click EDITION 5 to listen to this interview where Dr. Wright discusses Hamstring Injuries which are common in track & field, softball, baseball and water skiing, actually any sport that involves taking off into a sprint. Hamstring injuries vary in severity from a slight tear in the muscle belly to a complete rupture that would need surgical intervention. Understanding the anatomy of the hamstring helps understand the injuries.

What is a hamstring? The hamstring is a big muscle that runs down the back of your leg with a tendon on each end attaching it to the bone. It starts in your pelvis and inserts on the tibia in the lower leg. If the tendon pulls off the bone in the pelvis, for example in a water skiing injury, you will experience pain in the buttocks. This will most likely require surgical repair. In any sport or activity it is important to properly warm-up the body before participation to help prevent these types of injuries.

If surgery is necessary, he can perform a variety of open and arthroscopic surgeries at Missoula Bone & Joint’s outpatient surgery center on Mullan Road in Missoula, Montana. Arthroscopic surgery is used to relieve pain, improve joint stability, remove loose bodies within the joint, repair labral tears, and delay the onset of osteoarthritis and possibly delay the need for a total joint replacement. Arthroscopy is a surgical procedure that allows your surgeon to see the structures inside your joint utilizing small incisions and minimally invasive techniques. A small camera, called an arthroscope, is inserted into one incision and the surgical instruments are inserted into another incision. The camera displays images on a video monitor which your surgeon uses in real time to guide the surgical repair. Common arthroscopic surgeries include rotator cuff repair, ACL knee reconstruction, shoulder labral repairs, hip labral repairs, knee meniscus repairs, to name a few.
